If you would like a mailed application, please call the Championship Office. If you would like to pay by check or cash, you must fill out the mailed application and send it with your payment to the Championship Office.

Once your application has been processed, you will be contacted by your committee chairman to begin scheduling. Please allow 3-4 weeks for this process.
